What exactly are you trying to do? I use Carrara and Zbrush together (though not as much as I'd like to!). Why do you want to send a model from Carrara back to Zbrush? Are you posing the lowest-subdivision model in Carrara and then reimporting it to repose the Zbrush model?
The main thing I know about Zbrush's UV mapping is that, when knocking .obj files back and forth between applications, you need to be certain that the vertex order remains the same, and that you import and export at the right subdivision resolution. The main thing I know you should do is keep the entire model as a single object when going between programs - breaking it into individual elements definitely screws up the vertex numbering...
