Not sure what is causing this. I've had similar issues. I'd report it as a bug.

How did you manually re-zero the vertices? The best technique for that is to select the side and scale down on the axis you want to zero too.

I'd adjust the base mesh a little and go from there. Not seeing the weird shapes exactly. Any chance of a side view?
